When is my claim due?
The claim is due on the 5th of the month.

Can I claim my own children?
Yes, if you are Tier I and you are tending at least one that is not your own

How many hours of training are required?
2 hours are required by the Federal Government every year.
 
How many visits will be done in a year?
A minimum of 3 visits are required but could be more.
 
When are enrollments due?
If you are doing WEB HX, enrollments must be in our office before you submit your claim and; if you are doing the bubble attendance form, enrollments must be in our office no later than the the claim.

What information do I need to update every year?
All FDCH, IEF and Re-Enrollment Forms.
 
What do I need to do if I claim weekends?
You need to send a parent's note with the date, times, and parent signature.
 
What do I do when I plan a field trip and/or picnic?
You need to call the office with the date and time that you will be gone. You also need to let us know how you will maintain the food hot or cold.
 
Are sign-in-and-out sheets optional or mandatory?
Sign-in-and-out sheets are mandatory by the Food Program and Licensing.
 
How long do I have to keep paperwork from the Food Program?
You need to keep paperwork from the Food Program for 5 years.
 
How long can a substitute take my place?
The substitute can take your place for a maximum of 10 days and you must notify our office.
 
What do I do when I am planning to take time off and/or vacation?
You must notify our office with dates.

How do I obtain a license?
Call the Bureau of Licensing at 801-538-4034.
 
Does my license remains valid if I move to another location?
Your license loses validity imadiately upon moving.

Why should I keep my check stubs?
You should keep your check stubs because we do not provide a W-2 form since this is a reimbursement.
